How to write (good) intended outcomes

One of my least favorite things to do at work used to be writing intended outcomes. Twice a year, I would sit down and try to cobble together a list of goals that I would then use to talk about my growth with my manager. There are a variety of formats out there, but regardless of what I tried, they all seemed disconnected and, well, a little pointless.
